Who is eligible for 'My Wish'?
Any individual with a terminal diagnosis and a life expectancy of six (6) months or less.
Who can make a referral to the program?
Medical professionals, including MD, RN, NP, social workers, and paramedics, family members, or parent/legal guardian.
Recipients must be capable of experiencing the Wish. Recipients must have approval from their physician. A release form from their health care facility may be necessary.
Who decides if my loved one will receive a wish?
Heartland Hospice Moose Jaw has a team of delegates that will review all wish requests in order to determine if the potential recipient is eligible, if the wish request qualifies for the program, and if HHMJ or its partners can coordinate the wish. Will the experience and information remain private?
No photos or statements will be released to the media, on our website, or on social media without consent of the family and/or patient, with permission given via the Heartland Hospice Moose Jaw Media Release Form.
Heartland Hospice Moose Jaw does ask, within 30 days of the wish experience, that families provide a written summary of the wish experience, with photos or video if possible, to your Heartland Hospice Moose Jaw delegate. If you will allow us to use the story, please attach a copy of the Heartland Hospice Moose Jaw Media Release Form.
